All students at Our Savior’s Christian School are required to comply with New York State health regulations. Families must submit a current physical examination form completed by a licensed healthcare provider within the past year, along with a complete and up-to-date immunization record, prior to the start of the school year. In accordance with New York State law, students must meet all vaccination requirements to attend school, unless a valid medical exemption is on file. These forms help ensure the health and safety of our school community and must be updated as required by the state and the school throughout the year.
