All students are required to submit a New York State School Health Examination (physical) form and an up-to-date immunization record as part of the application process. A blank NYS physical form can be accessed using the link below. Please note that New York State requires physicals for students entering Pre-K/Kindergarten, Grade 1, Grade 3, Grade 5, and Grade 7.
