Medical Forms & Health Requirements

To comply with New York State regulations and ensure the health and safety of all students, the following documents are required for enrollment:

New York State School Health Examination (Physical) Form – completed and signed by your child’s healthcare provider

Up-to-date Immunization Record – must meet current New York State requirements.

Required Physical Exam Grades – NYS mandates updated physicals for students entering:

• Pre-K/Kindergarten
• Grade 1
• Grade 3
• Grade 5
• Grade 7
• Grade 9
